For the killing of woman from Elk Grove Village:

I don't get it: they make bad infrastructure worse to avoid killing of bicyclists?
I wouldn't expect drivers who don't look on "normal" intersections to make the extra look needed to detect bicyclists on separate ways. But maybe building this stuff takes the "right of way" from the bicyclists so that in future the killing of a bicyclist will always be the bicyclists fault...
To route bicyclists straight over an intersection parallel to the MIV which may turn right is another idiocy (if I understand correctly what you mentioned is going to get built there)
To picture this: http://radwege.udoline.de/pfeile.svg
It is even worse when the bicycle lane is offset by some meters and the bicyclists will be hidden by cars, hedges or shrubs like here:
http://bikeportland.org/2012/10/12/pbot-begins-re-design-of-ne-multnomah-now-with-more-protected-bikeways-78743
Sure I'd like to know why bikportland never published my critical comment to that lane, but I guess that will never happen.
It seems everybody and every country has to repeat the errors others made already to learn themselves...

What (imo) would help:
raise the sentences
make clear for drivers that bicylists are traffic as cars are (though I don't know how to teach this to some of the more stubborn)
And [as you say yourself] draconian speed limit enforcement.
